Preschool
Submit copies of the following documents with your registration form:

1st Grade
Submit copies of the following documents with your registration form:
- Birth Certificate
- Immunization records
- Most recent report card
- Baptismal Certificate
- Most recent standardized test results
**All incoming first graders are given a screening test prior to final acceptance.
Kindergarten
Submit copies of the following document with your registration form:
- Birth Certificate
- Baptismal Certificate
- Immunization Records
**All incoming Kindergarten students are given the Brigance screening prior to final acceptance.
2nd–8th Grade
Submit copies of the following documents with your registration form:

- Most recent standardized test results
**Contact the school to set up a date for required shadowing and testing. Tests will only be administered to students submitting completed applications.

Please note the $25.00 application fee is non-refundable. Current St. Augustine Catholic School families, please contact Admissions prior to completing a new application for an additional sibling to waive the application fee.

Application Process
- Complete the Admission Application online. If you are new to the SchoolAdmin Parent Portal (link), you will first need to create an account.
- $25 non-refundable application fee; paid online at the time of application submission.
- A copy of your student’s birth certificate – upload to the application portal.
- A copy of your student’s Baptismal Certificate (if applicable) – upload to the application portal.
- Your child’s most recent report card as well as the most recent standardized testing – upload to the application portal.
- Parish Verification Form – required in order to receive the CCASTA parish discount (if applicable).
- Schedule a required prospective student screening for students entering grades K-8. 2nd grade through 8th grade will receive the MAP screening. Kindergarteners and 1st grade students will have Brigance screening.
- Schedule a required shadow day for grades 2 – 8. Students will join us for the day and parents are asked to pick up their child by 2:45pm.
Preschool – 1st grade students will schedule a drop in classroom session (parents are asked to stay).
