Maryland Department of Health Blood Lead Testing Certificate

This form needs to be completed if your student was born after January 1, 2015.

Maryland State Department of Education Office of Child Care – Allergy and Anaphylaxis Medication Authorization Form

This form is required for any child that requires an epi-pen at school.

Maryland State Department of Education Office of Child Care – Medication Administration Authorization Form

This form is required for any child that requires medicine at school including over-the-counter medication.

All forms will need to be downloaded, completed and then uploaded to the parent portal, before August 1st for the upcoming school year.
